THE VITAL LINK SOLUTION:

Vital Link is a web-based and mobile application that provides a communications platform capable of sending and receiving securely encrypted messages between any PC or mobile device. It is used not only for clinician-to­ clinician communications between smart-phones, but also as an effective outreach to patients and families during and after a healthcare event. Using Vital Link’s Patient Engagement module, practitioners can:

  • Engage patients at point of delivery with a visual display of their record and care plan, supporting better understanding and partnership in future care plan developments;
  • utomatically deliver post-discharge instructions/educational materials to patients and/or advocates after they are comfortably at home and in a more teachable frame of mind to absorb and respond;
  • Send ongoing, scheduled follow-up notices (prescription renewals, outpatient appointment reminders) to patients post-discharge using email, auto­ outcall, and/or SMS;
  • Send regular status updates to patient family members and advocates during and after a surgery , relieving anxiety and creating a better bond between practitioners, patients, and the patient’s support system;
  • Deliver electronic Patient Satisfaction surveys following any healthcare event. Vital Link compiles and stores responses in a database for report generation and compliance;
  • Include within patient communications an invitation and links encouraging use of the patient portal for information and two-way communication with practitioners.
